About The Position

The Care Continuum Coordinator (CCC) helps clients and families navigate our services to ensure smooth transitions and consistent support. This role focuses on connecting people to the right care at the right time—whether that means adding personal care services, moving from home health to hospice, or returning to care after a hospital stay. The CCC works across our systems to monitor active cases, identify changing needs, and coordinate with internal teams so clients experience seamless, timely service. Salary range from 72k-90k

Requirements

  • 3+ years in care coordination, case management, or a similar role.
  • Strong understanding of how home-based services work and when they’re needed.
  • Comfortable using technology and multiple systems.
  • Excellent communication and organizational skills.
  • Valid driver’s license; ability to travel locally.
  • Bachelor's degree in Business Administration or related field, CPA and Master’s Degree preferred.
  • Has at least three to five years of experience in health care.
  • Information systems knowledge required.
  • Demonstrated ability to supervise and direct professional and administrative personnel.
  • Knowledge of corporate business management, governmental regulations and standards
  • Demonstrates good communications and public relations skills.

Responsibilities

  • Service Transitions: Help clients move between Home Health, Hospice, and Personal Care Services without interruptions.
  • Hospital & Facility Coordination: Stay in touch with hospitals and rehab centers to ensure clients return to care quickly after discharge.
  • System Oversight: Use agency systems to track cases and update progress so everyone stays informed.
  • Communication & Support: Be a resource for clients, families, and team members—explaining options and making sure expectations are clear.
  • Collaboration: Work closely with care teams and schedulers to align plans and timelines.
