About The Position

The Community-Based Care Coordination grant provides one year of funding for two positions. While Asotin County Health District (ACHD) intends to do all they can to continue funding for the position, we cannot guarantee it will continue beyond December 31, 2026. The Community Resource Navigator (CRN) will work to eliminate or mitigate barriers to health and social services for community members by serving as an advocate and link to connect clients with resources and services. They will also assist clients in accessing these resources, providing services in and around the community, as well as in the office.

Requirements

  • High School Diploma or GED
  • Valid U.S. driver's license
  • Excellent written and verbal communication skills, including the ability to modify language to meet clients' needs
  • Ability to work with people who may have different lifestyles and priorities than your own
  • Ability to work independently and collaboratively
  • Ability to learn and use computer programs
  • Ability to retain confidential information; ensure the security of sensitive data
  • Ability to act with integrity and professionalism
  • Excellent relationship-building skills
  • Excellent ability to set firm boundaries with clients while maintaining a high level of service
  • Criminal background check upon hire

Responsibilities

  • Maintain a regular caseload of clients who qualify for this assistance
  • Compliance with program grants and contract
  • Work out in the community, to be readily available and accessible to clients
  • Serve as a client advocate and liaison to reduce barriers to care
  • Refer clients to appropriate resources based on client-identified health and social service goals
  • Assist clients with resource navigation such as completing applications or finding transportation
  • Establish trusting relationships with referred clients and community partners
  • Maintain ongoing assessment of client and family needs to meet expected outcomes
  • Attend regular staff meetings, trainings, and other meetings as required
  • Other duties as assigned
  • The employee filling this position may be required to stay at or return to work during public health incidents and/or emergencies to perform duties specific to this classification or to perform other duties as requested in an assigned response position. This may require working a non-traditional work schedule or working outside normal assigned duties during the incident and/or emergency.
