About The Position

At Home Harmony is redefining care for aging adults through a coordinated, home-based medical and pharmacy model. Our Care Concierge Team—comprised of experienced RNs, LPNs, and MAs—is passionate about delivering high-touch clinical support and thoughtful case management. As a Care Concierge / RN , you'll partner closely with a local provider to manage and support a dedicated patient panel. This is a hybrid role , blending remote clinical work with occasional in-office and in-home patient support. Your first 90 days will be office-based for training, followed by a flexible mix of home, office, and limited community-based work. Requirements

  • 5+ years of experience in case management, home health, hospital, or outpatient settings
  • Experience in geriatrics, family/internal medicine, cardiac, or pulmonary care
  • Active, unrestricted RN license and BLS certification
  • EMR experience (Athena preferred) and strong technology skills
  • Excellent communication, organization, and documentation skills
  • Self-motivated, adaptable, and comfortable working independently in a remote environment
  • Flexible and adaptable to changing operations as we grow and develop this office
  • Valid drivers license, clean driving record, and ability to lift up to 50 lbs

Responsibilities

  • Serve as the primary clinical liaison between provider, patients, families, and care partners
  • Triage patient concerns via phone and manage EMR inboxes
  • Schedule appointments, document care, and maintain accurate EMR records
  • Coordinate diagnostic testing, DME orders, and review results for provider follow-up
  • Monitor remote patient data (BP, pulse ox, etc.) and respond to abnormal readings
  • Develop personalized care plans and support chronic care management
  • Assist with care transitions (hospital discharges, rehab admissions, home health coordination)
  • Educate patients and caregivers on disease management and risk reduction
  • Set up RPM devices and complete assessments as needed
  • Perform infrequent home visits for lab draws or patient support
