Responsibilities

• Complete program outreach and friendly visits (telephonic and in-person) to clients.
• Assist clients with light household chores and personal errands, such as grocery shopping and transportation.
• Document direct care visits of clients, ensuring compliance with program regulations.
• Manage relationships with multiple stakeholders in the community.
• Provide direct client services under the supervision of certified staff members.
