About the position

The Care Coach is a nationally certified medical assistant responsible for working directly with patients to support self-management of chronic conditions. This role involves conducting one-on-one encounters and group sessions, monitoring care plan goals, and collaborating with healthcare teams to ensure comprehensive patient care. The Care Coach plays a vital role in promoting health education and lifestyle modifications to enhance patient outcomes.

Responsibilities

  • Promote Patient Care Plan goals and reinforce lifestyle modification.
  • Conduct patient face-to-face visits and group workshops within their assigned center.
  • Ensure timely visits for patients with Chronic Conditions in their preferred learning mode.
  • Collaborate with the Center to close care gaps and ensure preventive care is completed.
  • Participate in interdisciplinary care team meetings and communicate with providers.
  • Document patient information accurately in the EMR system.
  • Participate in health fairs and health promotion events.
  • Build trusting relationships with patients and identify those who will benefit from care education programs.
  • Complete a care plan for each patient enrolled in the Care Program.
  • Create a personalized plan of care based on chronic conditions and lifestyle changes.

Requirements

  • High school diploma or equivalent.
  • Graduation from a nationally accredited Medical Assistant (MA) program.
  • Two years of Medical Assistant experience in a primary care setting.
  • Active MA national certification.
  • Certified Clinical Health Coach certification (to be obtained within the first 4 months of employment).
  • CPR/BLS for Healthcare Providers certification.
  • Experience in Microsoft Word, Excel, and PowerPoint.
  • Excellent oral and written communication skills.
  • Detail-oriented with strong problem-solving abilities.
  • Ability to manage stressful and fast-paced work environments.

Nice-to-haves

  • Experience working with chronic disease education management.
