About The Position

Provides social work and discharge planning services including psychosocial assessments, brief patient/family counseling, support, resources, education and intervention. Arranges and communicates efficient, timely and cost-effective discharge plans.

Requirements

• Current unencumbered Oregon RN Licensure required.
• BSN preferred.
• Three (3) years RN experience required.
• Healthcare Provider Level BLS required.

Nice To Haves

• Three (3) years hospital nursing experience preferred.
• Acute care utilization review, discharge planning or case management experience preferred.
• Utilization Review or other inpatient Case Management certification preferred.
• Experience or training in insurance benefits or covered services including Medicare and Medicaid preferred.
