About The Position
Cheer Home Care is hiring a Care Case Coordinator in San Diego. This role involves collaborating with interdisciplinary team members to develop and implement individualized care plans. The coordinator provides constant feedback to clients, client representatives, and other healthcare providers, working closely with the office team. The PT Care Case Coordinator delivers interventions, including individual treatment plans, focused on improving mobility, strength, and independence for clients.
Requirements
- Active California Physical Therapist License
- 3 years of experience as a Physical Therapist
- Proof of Negative TB result
- Physical Screening Documentation within the last year
- Valid California drivers license and car insurance
- Willing to work one weekend a month as back-up on call
- Ability to multitask, drive, take action
- Strong Organizational & Communication Skills
- Cell phone with internet access

- Home Health experience preferred
