About The Position

Provides personal care services to clients in their residence as directed by the Care Coordinator. Essential Duties and Responsibilities: Teamwork with the following and all other duties and responsibilities assigned. Provides primary care including all ADL’s, including but not limited to bathing, dressing, toileting, restorative nursing, and vital signs. Monitors the safety of the client. Provides restorative care for clients in their residence as per individual needs, physician order and support plan. Attends meetings with other staff to discuss client needs and to participate in utilization review, case management, and quality assessment activities and analysis. Attends other meetings as required or appropriate for position Maintains on going communication with the Care Coordinator regarding client needs. Other Responsibilities: Demonstrates respect and regard for the dignity of all clients, families, and fellow employees to ensure a professional, responsible, and courteous environment. Initiates immediate intervention in abusive situations to protect the client and report allegations according to facility policy. Notifies Care Coordinator and families when there are changes in condition and emergencies.

Requirements

  • Must be at least 25 years of age
  • High School diploma or equivalent
  • Complete annual state mandated training requirements (Regular In-services as well as any external training).
