About The Position

Requirements

• Bilingual proficiency in English and Spanish.

• Familiarity with Medicare and Medicaid programs.

Responsibilities

• Conduct welcome calls and health risk assessments for new members.
• Educate members on the Medicare Care Teams and their utilization.
• Inform members about routine health screenings and requirements of the DSNP program.
• Assist members in navigating CHPW benefits and connecting them to appropriate teams.
• Outreach to members to close care gaps and facilitate appointments with providers.
• Provide telephonic triage for inbound calls from members, families, and caregivers.
• Support members in crisis and activate the Crisis Caller in Distress process.
• Receive warm handoffs for Medicare members needing health risk assessments or care team access.
• Distribute educational materials to members as directed by the Medicare Care Team.
• Participate in Medicare Care Team meetings and workflows.
