What You’ll Do As the Cardiometabolic Health Program Lead, you will drive the implementation of a comprehensive strategy to improve cardiometabolic health standards of care across rural Indiana. You will coordinate with healthcare providers, public health organizations, and community partners to establish evidence-based clinical protocols, expand screening and intervention programs, and measure outcomes against CMS performance targets. This role requires balancing strategic program oversight with hands-on coordination, ensuring that clinical standards translate into improved patient outcomes in resource-constrained rural settings. Requirements

  • Bachelor’s degree in public health, health administration, nursing, nutrition, or exercise science; master’s preferred
  • 5+ years of experience in chronic disease prevention, cardiometabolic health, or community-clinical integration
  • Experience designing or implementing evidence-based health interventions
  • Strong understanding of rural health systems and community health models
  • Demonstrated ability to manage complex projects with multiple stakeholders
