About The Position

The Cardiology Nurse Navigator plays a vital role in improving cardiovascular care for our rural community by guiding patients across the continuum of care—from diagnosis through treatment and follow-up. This position serves as a clinical expert, patient advocate, and care coordinator, ensuring timely access to services, improved outcomes, and exceptional patient experience in a Critical Access Hospital setting. This role is ideal for a highly independent, compassionate RN who values building long-term relationships with patients and providers and making a meaningful impact in a close-knit community.

Requirements

  • Current Registered Nurse (RN) license in the State of [State]
  • Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN)
  • Minimum of 3 years of nursing experience, preferably in cardiology, critical care, emergency, or medical-surgical nursing
  • Strong clinical assessment, communication, and care coordination skills
  • Ability to work independently and manage multiple priorities

Nice To Haves

  • BSN strongly preferred
  • Cardiology experience in a hospital or clinic setting
  • Certification such as CCRN, CMC, CSC, or Cardiac-Vascular Nursing (CVRN)
  • Prior experience in care coordination, case management, or navigation
  • Familiarity with rural healthcare delivery and Critical Access Hospital operations

Responsibilities

  • Serve as the primary point of contact for cardiology patients, families, and caregivers
  • Coordinate cardiac care across inpatient, outpatient, diagnostic, and specialty services
  • Educate patients on cardiac conditions, procedures, medications, and lifestyle modifications
  • Facilitate timely referrals, testing, procedures, and follow-up appointments
  • Collaborate closely with cardiologists, primary care providers, nursing staff, and ancillary departments
  • Identify and address barriers to care, including transportation, health literacy, and access to services
  • Monitor patient progress, adherence to treatment plans, and outcomes
  • Support transitions of care to reduce readmissions and improve continuity
  • Assist with quality improvement initiatives, population health efforts, and cardiac program development
  • Maintain accurate documentation in the electronic health record and track required metrics
