About The Position

The Cardiology Nurse Navigator functions as a member of the Cardiology team at Western Missouri Medical Center, including inpatient and outpatient cardiology patients. The Cardiology Navigator’s primary function is to build a relationship with providers, staff and patients and to coordinate care for cardiology patients, especially heart failure patients needing education and outpatient follow up. The Cardiology Navigator will also cross train to support the cardiology service line, including patient access, scheduling, stress testing and other functions needed for service line success.

Requirements

  • High school diploma or equivalent.
  • Graduate of accredited school of professional nursing.
  • Currently licensed to practice as a professional Registered Nurse (RN) in the state of Missouri.
  • Current American Heart Association (AHA) BLS required.
  • Current American Heart Association (AHA) ACLS preferred.
  • Minimum of one-year professional nursing (RN) experience required.
  • Cardiology experience preferred.
  • Must be self-motivated and have the ability to work within the policies, procedures and practices established by the hospital/clinic

Responsibilities

  • Patient Assessment: Obtains and documents medical history, including chief complaint, risk factors, disease condition and medication reconciliation; Assesses and documents vital signs and pertinent physical exams.
  • Patient Education: Provides education to patients and their families regarding health status, acute and chronic medical conditions, care plan and treatments, medication management and results from testing; Provides education for and obtains patient consent for care, as applicable.
  • Patient Care and Monitoring: Provides care based on practice guidelines, standards of care, and federal/state laws and regulations; Executes prescribed treatments and interventions; Administers prescribed medications; Monitors patient progress and response to treatment.
  • Care Coordination: Collaborates with providers and multidisciplinary teams to develop and implement individualized care.
  • Documentation: Maintains and reviews patient records and other pertinent information; Documents patient assessments and interventions based on established guidelines, utilizing electronic health records (EHR) systems and/or approved medical record forms.
  • Communication: Responds to incoming patient phone calls based on clinic guidelines; Effectively communicates with providers regarding any concerns or issues, including testing and procedure results.
  • Patient Support and Advocacy: Serves as an advocate for patient concerns and care, including delegating as allowed under the Nurse Practice Act and consulting with providers and community resources to prevent/resolve issues and concerns.
  • Quality Improvement: Participates in multidisciplinary teams to contribute and improve patient care processes and outcomes; Tracks quality assurance data.
  • Confidentiality: Maintains patient confidentiality.
  • Attendance and Reliability: Maintains regular and predictable attendance.
  • Performs other essential duties as assigned.
