About The Position

Copley Hospital, located in Morrisville/Stowe VT, is looking for a Cardiopulmonary Rehabilitation Clinician to join our team! Work alongside a dedicated Medical Director and Clinical Rehab Director to deliver compassionate, evidence-based care. This is a Part-Time position (FT available). Copley Hospital has a variety of shifts available across Full-Time, Part-Time, and Per Diem needs with day and evening shift availability. Requirements

  • Bachelor's, Master's or Doctoral Degree in Physical Therapy from an accredited school for Physical Therapist
  • Bachelors' Degree in Exercise Science or Exercise Physiology with a focus on Cardiopulmonary Rehabilitation for Exercise Physiologist
  • Associates Degree in Nursing or Respiratory Care from accredited schools for Registered Nurse or Respiratory Therapist
  • Three years experience providing patient care services in a healthcare environment in cardiac and/or pulmonary rehabilitation, Acute care Physical Therapy, Respiratory Therapy or Cardiopulmonary Nursing
  • Current Vermont licensure without stipulations for Physical Therapist, Registered Nurse and Respiratory Therapist
  • Current BLS & ACLS Certifications, or certifications acquired within 6 months of hire

Nice To Haves

  • Master's degree in Exercise Science for Exercise Physiologist
  • Advanced training in Cardiopulmonary Rehabilitation preferred
  • Experience performing patient assessments, interventions, education, documentation, reporting and communications specific to Cardiac Rehabilitation and/or Pulmonary Rehabilitation
  • Use of EHR
  • Prior or current telemetry experience
  • Certification as a Clinical Exercise Specialist (ACSM)
  • Certification in Cardiopulmonary Rehabilitation from AACVPR

Responsibilities

  • Coordinate all aspects of the Cardiac and Pulmonary Rehab program
  • Provide direct patient care, including exercise prescription and education
  • Collaborate closely with physicians and interdisciplinary teams to develop individualized treatment plans
  • Monitor patient progress and ensure quality, safety, and compliance standards are met
  • Lead program development, documentation, regulatory compliance and outcome tracking initiatives
  • Provide group and individual patient and family education for established CR and PR education topics
  • Assess patient compliance with all medications based on patient reports, symptoms and clinical evidence
  • Assess need for and provide Tobacco cessation behavior interventions
  • Determine appropriateness of using Heart watch and teach patient its use
  • Participate in department and program staff meetings and quality improvement activities
  • Utilize the department computer system for telemetry monitoring and documentation
  • Follow department procedures and policies for patient registration and billing
  • Complete a comprehensive patient Initial Assessment and document in accordance with department policy
  • Manage exercise sessions safely while capturing all required documentation elements
  • Develop patient individualized treatment Plan of Care in accordance with department policy
  • Assess signs and symptoms of hyperglycemia and hypoglycemia and take appropriate actions
  • Compile & submit Evaluation and Plan of Care to referring providers and Medical Director
  • Complete and submit monthly Progress Reports for assigned patients
  • Compile and report patient blood lipid data in accordance with CPR standards
  • Complete and submit Discharge Reports for assigned patients
  • Communicate and interact with patients, families, colleagues, management & providers
  • Employ Critical Thinking skills to achieve outcomes and solve problems
  • Take personal responsibility for providing professional, organized & compliant care
  • Incorporate relevant safety and intervention skills in delivery of care
