The Cardiac Procedure Navigator is responsible for the provision and oversight of professional nursing care and medical care prescribed by the Licensed Healthcare Provider (Physician, Advanced Practice Registered Nurse (APRN), or Physician Assistant (PA)) involving the Structural and Surgical Heart program. This role assists in facilitating care coordination of multiple complex patients to ensure positive outcomes and effective overall plan of care. The navigator works closely with providers, hospital and clinic staff in clinical activities, provides patient/family education, and participates in hospital and clinic care coordination.

  • Works closely with physicians, clinic staff and hospital team to identify patients and provide care for the structural or surgical heart patient.
  • Coordinates the Structural Heart and Open-Heart Programs.
  • Receives physician referrals and efficiently compiles clinical data for presentation to a multi-specialty physician team at scheduled program meetings.
  • Performs clinical and social-economical assessment of referred patients and navigates care between physicians and diagnostic services.
  • Facilitates patient admissions, discharges and transfers within perioperative service line.
  • Takes patient care assignments or charge role during volume or acuity changes or during increased staffing demand.
  • Participates as a team member in the provision of health and medical care; interacts and collaborates with professional colleagues to provide comprehensive care.
  • Assists with accurate documentation of patient findings, treatment plans and evaluations; documents interventions appropriately to ensure coding compliance for reimbursement purposes.
  • Presents patients at monthly Transcatheter Aortic Valve Replacement (TAVR) meeting.
  • Monitors outcomes and works closely with the hospital clinical outcomes team to ensure accurate data for registry.
  • Works closely with inpatient and clinic staff to provide clinical guidance and education to hospital and clinical staff.
  • Performs independently and with team members to meet the physical and psychosocial dimensions of the patients through collaboration with nursing staff, physicians, and other health care professionals.
  • Provides support, when requested, to the Cardiology Inpatient Advanced Practice Provider (APP) service.
  • Anticipates and reviews caseload and surgical/cath lab schedule to plan for equipment and supply requirements for future cases.
  • Develops and manages systems for detecting clinical outdates on equipment, drugs and supplies; acts to correct any deficiencies found.
  • Utilizes the findings from quality improvement efforts to enhance patient care, customer satisfaction and cost-effectiveness of services.
  • Presents equipment requests, supplies and capital items to Nurse Manager and Cardiovascular Services (CVS)/Interventional Radiology (IR) and Perioperative Value Analysis teams as appropriate.
  • Works collaboratively with the Nurse Manager to ensure appropriate equipment function, equipment availability and efficient supply usage and charging.
  • Keeps Nurse Manager informed of day-to-day operational needs/issues.
  • Assists patient/family with navigation of testing schedule. Educates patient and family about tests/procedures, results, medications, nutrition and other health related issues.
  • Uploads required data to appropriate vendor and maintains communication with device representatives.
  • Participates in follow-up visits and coordinates further follow-up per registry requirements.
  • Complies with registry requirements and data support.
  • Graduation from an accredited Registered Nursing Program required.
  • Current Registered Nurse licensure from the State of Nebraska or approved compact state of residence as defined by the Nebraska Nurse Practice Act required.
  • Minimum of three (3) years nursing experience required.
  • Bachelors or Master of Science degree in Nursing preferred.
  • Cardiovascular nursing experience from a clinic, surgical or procedural role preferred.
  • Basic Life Support (CPR) certification required.
  • Advanced Life Support (ACLS) required.
