Job Description: Perform nursing cardiovascular assessments, establish patient goals, and implements plan of care. Evaluates outcome and makes recommendations based on needs of each Cardiac Rehab participant. Documents all vital signs and response to care provided in coordination with a multi-discipline care team. Plan, implement, coordinate, and conduct patient, family, and community teaching regarding CVD and smoking cessation. This teaching may occur in a hospital setting or in an outreach rural environment. Performs all required interface with County Government Officials, local agencies, businesses, and healthcare professionals for the purpose of activity/event coordination, arranging transportation and performance of health assessments. Functions in RN capacity, as needed, in the Cardiac/Pulmonary Rehab department.

Requirements

  • Graduate of approved school of professional nursing.
  • Valid NC or compact state RN license.
  • BLS and ACLS certified.
  • EKG Interpretation.
  • Demonstrate competence in the performance of duties specific to all age groups of patients.
