About The Position

Facilitates the coordination of care for patients through the continuum based on their diagnosis or procedure which may include but is not limited to ensuring follow up with physician offices, scheduling of necessary diagnostic procedures providing education and emotional support throughout the path of care or designated timeframe. Triages incoming referrals for acuity to assist with patient throughput. Educates patients about their treatments and assists with overcoming barriers to healthcare.

Requirements

• Associate's Degree in Nursing- required
• Must hold an active RN license- required
• BLS- required
• Minimum of three (3) years cardiology nursing experience- required
• Proficiency at triaging complex cardiology patients and understand general cardiology, interventional cardiology, as well as electrophysiology- required

Nice To Haves

• Bachelor's Degree of Science in Nursing- preferred
