About The Position

The position is for a full-time Cancer Care Coordinator at Advocate Health, primarily located in Grafton with travel to East Mequon on Fridays. The role involves providing continuity of care for patients diagnosed with cancer, ensuring smooth transitions between care settings, and personalizing approaches to maximize service management. Responsibilities include conducting nursing assessments, integrating care plans, guiding patients through diagnostic phases, and collaborating with various healthcare professionals. The coordinator will also provide education, support groups, and community outreach to promote cancer awareness and prevention.

Requirements

  • Registered Nurse license issued by the state of practice.
  • Basic Life Support (BLS) certification from the American Heart Association, to be obtained within 6 months.
  • Associate's Degree in Nursing.
  • Typically requires 5 years of experience in clinical nursing.
  • Working knowledge of quality and acute patient care needs.
  • Excellent analytical and interpersonal communication skills.
  • Demonstrated ability to work well with physicians and other professionals.
  • Ability to operate necessary equipment such as FAX, computer, and telephone.

Responsibilities

  • Provides continuity of care by ensuring smooth transitions between care settings according to critical pathways.
  • Develops relationships with patients and their primary care physicians to facilitate navigation through cancer diagnosis and treatment.
  • Completes nursing assessments focusing on disease site and identifies individualized patient needs.
  • Integrates with other departments to develop plans of care.
  • Provides evidence-based practice as a cancer care coordinator.
  • Guides patients with suspected cancer through the diagnostic phase.
  • Tracks testing and diagnostic reports to ensure timeliness.
  • Communicates necessary information and collaborates with healthcare providers.
  • Refers patients to appropriate resources such as financial counseling and clinical trials.
  • Acts as an advocate for patients with cancer.
  • Maintains contact with patients requiring ongoing support.
  • Facilitates support groups for patients and their families.
  • Provides in-services and education offerings to staff.
  • Counsels and educates patients and families on cancer prevention and early detection.
  • Evaluates and selects patient education materials.
  • Promotes awareness of specific cancer disease site programs.
  • Provides community education and outreach.
  • Partners with leadership to develop a vision for patient care.
  • Participates in the development of annual goals and objectives for program services.
  • Utilizes data to implement best practices and improve quality and efficiency.
