The purpose of this position is to serve as a therapeutic resource to consumers of A Caring Alternative in compliance with agency standards and best practice. Substance Use Disorder Services (SUD) provide structured individual and group addiction activities and are provided at an outpatient program designed to assist adult a time-limited, multi-faceted approach treatment service for adults to begin recovery and learn skills for recovery maintenance. These services are provided during day and evening hours to enable beneficiaries to maintain residence in their community, continue to work or go to school, and to be a part of their family life.

Requirements

  • Must be a CADC.
  • Exceptional written, oral, interpersonal, and organizational skills.
  • High level proficiency in Microsoft Office- MS Word, Excel, Access, and PowerPoint. Or an equivalent combination of education and experience
  • Resume must include degree earned (major & minor), graduation month & year, relevant employment dates including month and year, and populations served.

Responsibilities

  • Develop consumers’ PCPs and Crisis Plan, based on findings of the clinical assessment and consumer input
  • Ensure service authorizations are complete and entered correctly in the electronic medical record for acceptance of service note and billing submission in accordance to MCO utilization management requirements/restrictions.
  • Ensures completion of all intake/annual consumer documents, including service agreement, provider choice, release of information forms.
  • Provides case management to consumers, ensuring access to community resources and federal/state entitlements.
  • Develop and implement group materials and lessons plans.
  • Monitor group sessions for best practice; lead groups
  • Complete measurement tools, including SDOH, TOPPS
  • Assess consumer risk and develop safety plans
  • Monitor results of biochemical assays and, in conjunction with other team members, develop appropriate intervention for positive drug screens
  • Transition consumers as appropriate to other SUD services, Mental Health services, or discharge
