About The Position

HopeHealth Hospice and Palliative Care is seeking an RN who is passionate about end-of-life care. HopeHealth is working with Brigham & Women’s Hospital (BWH) in furthering its mission to provide excellence to their end of life patient population.  In this role, you will be at the forefront of innovation on how end of life is delivered and supported in an acute care setting. The Hospice Nurse Navigator role is ideal for a nurse clinician with well-developed clinical and assessment skills focused on disease progression, prognostication, and symptom management within an acute care setting. This is a full time 36 hours per week position, the hours are 9am-9:30pm

Requirements

  • Bachelor of Science Degree in Nursing highly preferred
  • Two or more years’ experience in hospice, palliative care and/or acute care
  • Nursing license in Massachusetts and Rhode Island required; Massachusetts license required upon hire and evidence of Rhode Island license application required prior to start
  • Individuals in this position will be required to achieve and maintain Hospice and Palliative Care certification through the Hospice and Palliative Nurses Association within three years of hire
  • Excellent communication skills

Responsibilities

  • Represents HopeHealth at BWH serving as a hospice liaison between staff, patients and families and is the subject matter expert in hospice
  • Provides initial assessment of the impact of the terminal diagnosis on the patient’s physical, functional, psychosocial and environmental needs and activities of daily living to determine eligibility for hospice
  • Presents hospice to patients, families and BWH staff in a clear manner highlighting the benefits of hospice in caring for individuals with a terminal diagnosis
  • Completes hospice admissions and initiates care in compliance with all Medicare and state regulations
  • Completes patient visits at least once a day to admitted hospice patients; helps coordinate and manage patient care with BWH nursing staff and communicates on a daily basis with a member of the BWH care team
  • Documents at point of care in both HopeHealth and BWH EMRs including conversations about patient care with the BWH teams
  • Uses interdisciplinary approach in developing plan of care, in accordance with HopeHealth policies; initiates referrals to other disciplines as necessary
  • Creates the individualized plan of care including documenting the Local Coverage Determination (LCD)
  • As required by Medicare and state regulations, periodically assesses patients continued eligibility for hospice services
  • Participates in weekly interdisciplinary team meetings to review patients receiving hospice inpatient care for 6 days or longer
  • Ensures that patient information regarding payer source is current and correct; works with BWH to obtain necessary insurance information and coverage
  • As appropriate, assists in discharge of admitted patients to a receiving facility
  • Compliance with BWH initial and ongoing credentialing, education and safety requirements
