Requirements

  • Experience in healthcare marketing, sales, or community outreach
  • Strong networking and relationship-building skills
  • Knowledge of home care or healthcare referral processes preferred
  • Organized, proactive, and able to manage multiple priorities
  • Comfortable using CRM systems and digital tools
  • Reliable transportation for daily field visits

Responsibilities

  • Build and maintain relationships with hospitals, assisted living communities, physician offices, and other referral partners
  • Conduct in-person visits, follow-ups, and regular check-ins to stay top of mind
  • Represent our agency at health fairs, networking events, and community outreach opportunities
  • Identify and develop new referral sources and partnerships
  • Track referral activity, leads, and conversion data in systems like WellSky
  • Collaborate with internal teams to ensure smooth onboarding and client experience
  • Monitor market trends and competitor activity to support growth strategies
  • Provide weekly/monthly reporting on outreach performance and results
