We are seeking a candidate with a strong positive attitude toward customer service to act as a MDS Coordinator (RN). Requirements

  • Minimum Registered Nursing degree.
  • MDS experience required.
  • Two years of experience working as a registered nurse including some medical/surgical experience and awareness of the needs of geriatric residents.
  • CPR

Responsibilities

  • Coordinate the IDT for Care Planning, assuring the highest quality of care for residents is developed at and above CMS guidelines.
  • Timely MDS completion per RAI manual. Giving special attention to completion of forms.
  • Timely completion of RAI process and RAP summary. Identification of problems, goals, and approaches.
  • Proper and accurate response to regulation changes and evaluation of the Care Plan system.
  • Assure timely signatures are obtained.
  • Effectively communicate with nurses and IDT on the current status of Care Plans.
  • Responds timely and in writing to deficiencies in MDS audits.
  • Assist with admission process.
  • Consistently demonstrate hospitality behavior and the core values of quality, respect, service and integrity.
  • Maintain work area and keep the department in compliance with OSHA safety standards.
  • Effectively communicates both verbally and in writing.
  • Demonstrates the community hospitality promises towards residents, families, associates and vendors.
