This role involves utilizing vetted and approved resources to establish a safe, effective care management plan, completing comprehensive psychosocial assessments, collaborating with various stakeholders, and identifying and initiating referrals to supportive services. The Social Worker will advocate for members, address age-specific communication needs, identify and intervene with high-risk members, maintain records, and participate in interdisciplinary team meetings. They will also act as a resource for complex members, understand insurance products and regulations, and protect member PHI in accordance with HIPAA standards. Professionalism and the ability to assess and respond to danger to self or others are also key aspects of the role.

Requirements

  • Undergraduate degree or equivalent experience
  • BSW - 1+ years of clinical experience in a healthcare setting, or successful completion of a relevant internship
  • Driver’s License and access to reliable transportation

Nice To Haves

  • Bachelor's degree from a CSWE-accredited School of Social Work or a bachelor’s degree in a Behavioral Science-related field

Responsibilities

  • Utilize vetted and approved resources to establish a safe, effective care management plan
  • Complete a comprehensive psychosocial assessment for each member and develop a member-specific action plan
  • Collaborate with members, families, and healthcare providers
  • Identify and initiate referrals to financial, psychosocial, community, and state supportive services
  • Advocate for members and assist them in navigating the healthcare system
  • Address age-specific communication needs and engage effectively with members and families from diverse backgrounds
  • Identify and intervene with high-risk members requiring social services
  • Maintain records of social care treatment plans, interventions, and outcomes
  • Participate in interdisciplinary team meetings
  • Act as a resource to department teammates, organization teammates, and providers for complex members
  • Understands insurance products, benefits, coverage limitations, insurance, and governing regulations as it applies to the health plan
  • Use, protect, and disclose members’ protected health information (PHI) only in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards
  • Portray a positive attitude and professionalism when interacting with others
  • Trained to assess and respond to danger to self or others in alignment with state licensure standards
  • Perform additional duties as assigned
