About The Position

Using a Housing First approach, Avalon's Support Coordinators provide outreach, intensive case management and housing support services to Avalon tenants and clients. The individuals we serve face many challenges, including mental illness, substance use disorders, and/or chronic medical conditions in addition to chronic homelessness. Our clients live in Avalon Housing units, with private landlords or in public housing located within Washtenaw County. Support Coordinators collaborate with community partners to coordinate delivery of a broad range of services that meet the mental, physical, psycho-social and housing needs of our clients, either through direct service or connections to appropriate resources.

Requirements

  • Bachelors or Master's degree in a social or behavioral science or relevant field or three years of relevant experience
  • Experience working with people who are homeless, people with mental illness, chronic health issues, and/or addiction disorders, or similar populations

Responsibilities

  • Provide direct case management and/or coordinate with case managers in the community to provide continuity of service for individuals and families we serve
  • Transporting clients and being on-site is required per our contract with AAHC, except on designated snow days or when actively transporting a client
  • Ongoing, active outreach to tenants/clients
  • Comprehensive tenant assessments and action plans to achieve goals
  • Ongoing case management for clients, including maintenance of clinical records and progress notes
  • Collaboration with property management to coordinate eviction prevention efforts
  • Provide crisis intervention as needed
  • Assist tenants with accessing needed medical, mental health, substance use, and psychosocial supports
  • Assist tenants in developing basic life skills including maintaining an apartment
  • Provide budgeting/financial literacy support
  • Link tenants to employment and skill development opportunities
  • Build community and peer support among tenants including participation in community programming activities
  • Meet documentation requirements as dictated by program need
  • Effectively build relationships with local community and external agencies
  • Provide support with pantry operations at State Cross
