Children’s Home Society of South Dakota is seeking a Full Time Bright Start Nurse Home Visitor to help provide comprehensive community health services for high-risk women and children. Requirements

  • Minimum of an Associate Degree in Nursing (ADN) required; Bachelor of Science in Nursing (BSN) strongly preferred.
  • Must possess and maintain a valid, unencumbered nursing license in the State of South Dakota and a valid driver’s license.
  • Previous experience in women’s health care, pediatrics, home visitation services, assessments and case management preferred.
  • Regularly in normal office environment. Area travel and client home visits are regularly required.
  • This position often requires lifting infants and children. The employee must regularly lift and/or move up to 10 pounds and occasionally lift and/or move up to 50 pounds.
  • Must successfully complete all required background and pre-employment checks

Responsibilities

  • Plan and facilitate home visits with program participants in accordance with Bright Start and Nurse Family Partnership principles.
  • Assess participant needs as defined in the protocol, setting measurable and achievable goals and plans.
  • Provide appropriate referrals to community resources.
  • Lead and support marketing efforts to represent the program effectively within the community.
  • Obtain and record client data according to Nurse Family Partnership model and Agency protocol.
  • Complete charting and documentation of client visits and interactions in a timely manner.
  • Complete required training.
  • Participate in Continuous Quality Improvement projects.
  • Must be able to be committed to delivering exceptional quality, fostering a culture of continuous improvement, and ensuring the safety and well-being of children and families in all aspects of our work.
