About The Position

As a Brain Injury Reentry Specialist, work face-to-face with clients transitioning out of legal system involvement in Denver specialty courts to develop an understanding of their brain injury symptoms and support them as they reintegrate into the community.

Requirements

  • Bachelor’s degree, or equivalent experience, in a related field.
  • Experience navigating systems in human service, case management, social work, legal systems, or other applicable settings.
  • Experience with small group facilitation.
  • Travel to other meetings and events within the state, sometimes after hours and on weekends.
  • Have the ability to read and interpret documents. Have the ability to write routine reports and correspondence. Have the ability to speak effectively in front of groups. Have the ability to engage in email regularly, with or without reasonable accommodations.

  • Bachelor’s degree in a related field.
  • Experience working with individuals who have cognitive disabilities and/or behavioral health challenges, as well as involvement in the legal system.
  • Experience teaching life skills, including organization, financial management, healthy lifestyle, social skills, job skills, and computer and internet use.
  • Knowledge and experience applying person-centered and culturally competent practices in interactions with clients, co-workers, professionals, and other community members.
  • Experience using a database tracking system (Salesforce preferred).

Responsibilities

  • Client Contact: be responsible for managing a caseload of BI survivors from 1-2 courts (Dockets TBD), ranging from 25-35 survivors, and offering case consultation to professionals.
  • Brain Injury Screening: Screen at least 100  legal system-involved individuals for brain injury annually (independently or in collaboration with court partners), using the Ohio State University Traumatic Brain Injury Identification Method or the Online Brain Injury Screening and Support System, as appropriate for the specific setting.
  • Re-entry planning: Collaboratively develop comprehensive and wrap-around re-entry plans with at least 100 legal system-involved survivors of brain injury annually.
  • Symptom Support Plan Development: Collaboratively develop symptom support plans and wraparound service plans with at least 100 Legal systems-involved survivors of brain injury annually.
  • Psychoeducation Groups: Facilitate weekly AHEAD groups for legal system-involved survivors of brain injury.
  • Client Support: provides wraparound services to help clients stabilize as they transition into a new phase of their lives.
  • Contact professionals who support survivors’ symptoms to share Symptom Support Plans. Offer case consultation and implementation support, strategies as appropriate to meet client needs.
  • Contact family members, caregivers, and other support network individuals (as ROIs allow) with survivors’ symptom support plans.
  • As appropriate, inform, refer, and help clients enroll in other BIAC services, including, but not limited to, counseling, the self-management program, recreation programs, classes and workshops, social activities, newsletter subscription, educational materials, financial support programs, and ticket giveaways.
  • Follow up with clients who are not eligible for this program and refer them to the proper BIAC program.
  • Within 72 hours of interacting with the client, update the case with the area of need and description, and record details of client contact related to the case, including duration, type of contact, and summary of activities.
  • Provide guidance, support, and hope to clients navigating the legal system and preparing to reintegrate into the community.
  • Regular, reliable, and predictable attendance is an essential function of this position. Because consistent presence is required to support brain injury survivors and meet obligations to court partners, employees must be able to perform this function with or without reasonable accommodation
  • Strengthen connections within the program area and departments: Attend and actively participate in scheduled department and organization meetings.
  • Lifelong Learning: Participate in training and professional development activities on an ongoing basis.
  • Attend between 70 – 110 hours of training and/or professional development annually. The supervisor will select up to 50 hours annually, and the employee will select the remaining hours.
  • Performs other related duties as assigned.
