About The Position

Core Wellness Home Care LLC is seeking a compassionate and dedicated caregiver to provide assistance and support to individuals in need. The caregiver will be responsible for assisting clients with activities of daily living, ensuring their safety and well-being, and promoting independence and dignity. This role requires excellent communication skills and the ability to work effectively in the client's home.

Requirements

• High school diploma or equivalent.
• Previous experience of 1+ years in caregiving, healthcare, or related fields preferred.
• Excellent communication and interpersonal skills.
• Compassionate, patient, and empathetic demeanor.
• Ability to work independently and as part of a team.
• Reliable transportation to be at client's homes as scheduled and expected.
• Flexibility to work various shifts, including evenings, weekends, and holidays.

Nice To Haves

• Certified Nursing Assistant (CNA) certification or equivalent training is a plus but not required.
• CPR and First Aid certification are a plus but not required.

Responsibilities

• Provide personal care assistance to clients, including bathing, grooming, toileting, and dressing.
• Assist clients with mobility, transferring, and positioning to ensure their comfort and safety.
• Provide medication reminders according to the instruction/schedule provided by the client or client advocate.
• Prepare and serve nutritious meals and snacks according to dietary needs and preferences.
• Perform light housekeeping tasks, such as tidying up living spaces, doing laundry, and maintaining a clean environment.
• Provide companionship and emotional support to clients by engaging in conversation, activities, and leisure pursuits.
• Monitor and report changes in client's condition, behavior, and vital signs to the family or client advocate.
• Assist clients with exercises, rehabilitation, and physical therapy activities as directed by client, client advocate, or health professional.
• Follow care plans and instructions provided by healthcare professionals and family members.
• Maintain accurate and up-to-date documentation of care provided, including daily activities, observations, and incidents.
• Respect and maintain client confidentiality and privacy at all times.
• Participate in ongoing training and education to enhance caregiving skills and knowledge.
• Collaborate with other caregivers, healthcare professionals, and family members to ensure coordinated and comprehensive care for clients.
• Adhere to safety and infection control protocols to prevent accidents, injuries, and the spread of illness.
