About The Position
Blended Case Management Supervisor Position Description: We are looking for a Blended Case Management Supervisor to join our Team! Nulton Diagnostic & Treatment Center is accepting applications for a Blended Case Management Supervisor for our Cambria County teams. The successful candidate will provide Blended Case Management Services and Supervision in accordance with Federal, State, and Agency requirements.
Requirements
- A. A master’s degree in social work, psychology, rehabilitation, activity therapies, counseling or education and 3 years mental health direct care experience; or
- B. A bachelor’s degree in sociology, social work, psychology, gerontology, anthropology, history, criminal justice, theology, counseling, education, or be a registered nurse, and 5 years mental health direct care experience, 2 of which shall include supervisory experience; or
- C. A bachelor’s degree in nursing and 3 years mental health direct care experience.
- The successful applicant must have a valid Driver's License and be able to obtain Act 33, Act 34 and FBI clearances.
