Primary Purpose: Obtains all necessary information for birth certificate completion through customer service oriented inquiry of patient. Performs birth certificate registration and acknowledgement of paternity process within approved practices and deadlines as regulated by the Texas Department of State Health Services and the Texas Attorney General's Office.
