This role involves assisting Medicare patients with chronic conditions by reinforcing care plans, promoting medication adherence and preventive care, and educating them on health management. Clinical Navigators engage in proactive, patient-centered outreach, including structured check-ins, coaching, goal-setting, care coordination to address gaps, and resolving barriers like access to resources or medication refills. They are responsible for documenting patient interactions in the Cadence platform and collaborating with the clinical team to escalate concerns and ensure patients feel supported and engaged throughout their care journey.
