About The Position

The Field Operations Coordinator plays a vital role in supporting the delivery of Health Homes Care Coordination services. This position is responsible for traveling to client residences to obtain signatures on essential care plan documents and other program forms. The Coordinator ensures smooth communication between clients and care managers while maintaining accurate documentation.

Requirements

  • High school diploma or equivalent required.
  • Excellent customer service and interpersonal skills.
  • Strong verbal and written communication abilities.
  • Effective time management and organizational skills.
  • Experience in healthcare, human services, or client-facing roles is a plus.
  • Bilingual skills English/Spanish required.

Responsibilities

  • Facilitate client engagement by scheduling, confirming, and conducting home visits to collect required signatures on care plans and program documentation.
  • Provide basic guidance to clients regarding their care plans and respond to general inquiries.
  • Serve as a liaison between clients and care managers, communicating any client needs, updates, or concerns promptly.
  • Accurately document all client interactions and maintain timely and organized case notes in compliance with program standards.
