About The Position

The CGP Case Manager is responsible for the provision of crisis intervention, protective services, and intensive case management on a voluntary or involuntary basis to a caseload of mentally and physically impaired, abused, and/or neglected clients. Under the direction of a Masters' level supervisor, the individual will perform various duties including conducting assessments, developing service plans, and advocating for clients. The position involves working with clients who have been judged legally incapacitated and ensuring their needs are met through comprehensive service planning and regular support.

Requirements

  • Graduate of an accredited college or university with a Bachelor’s of Social Work, Sociology, Psychology, or Human Services degree, or equivalent.
  • Bilingual in Spanish and English.
  • 1+ years of Case Management experience is strongly preferred.
  • Demonstrated organizational skills.
  • Ability to travel to clients’ homes using mass transit and to negotiate any steps or barriers in clients’ residences.
  • Excellent organizational and time management skills.
  • Must be computer literate, including the ability to learn and operate software.
  • Understanding and appreciation of the roles of race, religion, ethnicity, sexual preference, and individual values as they relate to serving clients and families.

Responsibilities

  • Conducts initial assessments and monthly in-home assessments of the social and emotional needs of the client and his/her family.
  • Screens the client for program eligibility, conducts triage for emergency interventions, ascertains short-term needs, and provides appropriate information and referral services to clients, families, and collaterals.
  • Develops and implements written service plans to provide a full range of social services.
  • Arranges for direct provision of services such as homemaker, home health aide, public assistance, Medicare, Medicaid, emergency cash relief, legal aid, protective services, vocational placement, medical and psychiatric examination and therapy, housing, etc.
  • Disburses monies as issued through JASA relief funds or client financial management services to secure needed goods and services.
  • Reviews the guardianship court order and ensures that all court ordered services are in place.
  • Advocates on behalf of the client.
  • Refers to, and maintains a cooperative relationship with, other community agencies to meet the needs of clients.
  • Seeks appropriate guidance from the supervisor in situations requiring clarification and consultation especially for cases that meet the high risk criteria.
  • Represents and interprets the agency at community conferences and meetings.
  • Participates in training and unit meetings.
  • Completes required case records, reports and statistics within mandated timeframes, utilizing computers and mobile devices as required.
  • Handles special assignments and duties as assigned.
