Provides individual, and/or group work services to patients and/or their families in various health care settings. Utilizes psychosocial assessment skills to deliver social work services. We are seeking a dynamic, dedicated and seasoned bi-lingual (Spanish speaking), licensed clinician to work as a member of an interdisciplinary care team in a NYCDOE school-based satellite clinic of the Adolescent Division at Cohen Children’s Medical Center. The clinic is located within John Adams High School in Ozone Park, Queens. This SBHC is one of 5 sponsored by CCMC, and provides comprehensive medical, mental health and reproductive health services to adolescents attending the school. Our mental health program is treatment focused, trauma informed, and strength based. Our staff receive training through the Office of School Health in evidence-based practices. The primary responsibilities of the clinical social worker, are to conduct mental health assessments, provide individual therapy, and manage crises. In addition, our team implements innovative youth development and therapeutic groups, provides collateral family sessions and collaborates ongoingly with school and SBHC staff for care coordination. On site supervision is provided, as well as opportunities to connect, collaborate and case conference with the larger mental health team. This is a unique opportunity to work with teenagers who are self-motivated and self-directed in seeking mental health treatment and to work within a vibrant and passionate team.
