Position Summary Candidates must live in Florida and speak English and Spanish Improves quality products and services by using measurement and analysis to process, evaluate, and make recommendations to meet objectives. Coordinates the plan's prevention and wellness activities specifically designed to promote the importance and benefits of accessing timely preventive and well care through targeted member/provider education and outreach initiatives.

Requirements

  • Candidate must be bilingual in English/Spanish
  • Candidate must reside in Florida
  • Working knowledge of problem solving and decision making skills
  • Adept at collaboration and teamwork
  • Adept at growth mindset (agility and developing yourself and others) skills
  • 3-5 years' work experience
  • Access to a private, dedicated space to conduct work effectively to meet the requirements of the position
  • Dependents must have separate care arrangements during work hours, as continuous care responsibilities during shift times are not permitted.
  • High School Diploma or GED (REQUIRED)

Responsibilities

  • Conducts telephonic outreach to members to promote clinical gaps in care closure related to Quality HEDIS measures
  • Assists with establishing and implementing results-based programs and innovative initiatives for the Prevention & Wellness area, under general supervision.
  • Applies in-depth knowledge of the Prevention & Wellness field to offer feedback for decisions and innovation strategies that enhance organizational growth and visibility.
  • Develops, implements, and assures the ongoing coordination of designated prevention and wellness activities outlined within the quality improvement and work plan.
  • Evaluates, analyzes, reports on and records outcomes of the scheduled prevention and wellness interventions.
  • Organizes special or pilot health education interventions to target specific population initiatives.
  • Produces and distributes the quarterly community newsletters.
  • Coaches more junior colleagues in techniques, processes, and responsibilities.
