About The Position

Bilingual Prevention Case Associate New Alternatives for Children, Inc. (NAC) is an award-winning not-for-profit agency dedicated to serving children and families with medical complexities, chronic illnesses, and disabilities throughout the five boroughs. To assist in achieving this mission, NAC is currently seeking a Bilingual Case Associate to join its Special Medical Prevention Program. Our Prevention Program delivers a range of clinical, case management, and supportive services to families who are child-welfare involved and caring for children diagnosed with chronic or complex medical conditions. The goal is for families to remain intact while ensuring caregivers receive the appropriate support to meet their children's physical, developmental, and mental health needs. The Bilingual Case Associate will assist Case Planners with casework activities and administrative case-related tasks. The ideal candidate will be strength-based, trauma-informed, culturally responsive and committed to family preservation and resilience. This is a citywide home and community-based program, and the candidate must have the ability to work independently and flex their hours on weekdays to meet the needs of youth and caregivers. Our hybrid model supports work-from-home when staff are not in the field. Requirements

  • Bachelor's degree in social work or related field
  • Bilingual in Spanish language
  • Excellent written, verbal, interpersonal, and engagement skills
  • Excellent organizational skills and the ability to work independently

Nice To Haves

  • Prior experience working with families in a child welfare setting is preferred

Responsibilities

  • Accompany clients to community-based appointments including school meetings, medical appointments, public assistance, housing, etc.
  • Provide support, advocacy, and Spanish translation as needed.
  • Provide Spanish translation for monolingual clients interacting with non-bilingual agency staff.
  • Joint home visits with Case Planners to provide age and developmentally appropriate play with children while Case Planner speaks with parents.
  • Provide parent education to assist families with in-home structure and parent-child interactions.
  • Contact schools, doctors, in-home service providers, mental health, substance use and domestic violence programs to gather ongoing information on clients' service needs.
  • Assist clients in obtaining grants and benefits.
  • Assist with case management and case-related administrative tasks including filing, documentation, and data entry using the agency's electronic health record.
  • Participate in case reviews, individual and group supervisions, department meetings, and agency-wide training.
