About The Position

Provide care to patients in a full-spectrum family medicine practice that serves all ages and conditions. Participate in, and provide, after-hours call coverage, as assigned. Prescribe or administer treatment, therapy, medication, vaccination, and other specialized medical care to treat or prevent illness, disease, or injury. Order, perform, and interpret tests and analyze records, reports, and examination information to diagnose patients’ condition. Complete charting in Athena and required paperwork within 72 business hours. Maintain confidential patient information. Participate in the development/review of organizational policies and quality improvement initiatives. Participate in health center and provider meetings. Ensure appropriate communication with patients, staff, and external parties. Prenatal care is optional and may be performed if privileged.

Requirements

  • Bilingual Polish/English or Spanish/English required.
  • Graduate of accredited educational program with a master’s degree in nursing or doctorate degree in nursing or physician assistant degree.
  • Board certification in specialty must be kept current.
  • Unrestricted, active license to practice within the State of Illinois.
  • Active, current Controlled Substance license within the State of Illinois and federal DEA certificate, without restriction.
  • Up-to-date provider-level BLS CPR.
