About The Position

At North Range Behavioral Health, we believe that whole-person wellness begins by caring for both the body and the mind, because behavioral health is an essential part of overall health. Guided by the philosophy “Mind the Body, Mind the Mind,” our Integrated Care program bring behavioral health services directly into the primary care setting, reducing barriers to access and strengthening outcomes for the individuals and families we serve, Our Integrated Care team works side by side with medical providers to deliver timely, coordinated behavioral health support where patients already receive their medical care. Services include behavioral health screening and assessment, brief interventions, case management, and individual and group therapy, all offered within the patient’s primary health home to ensure seamless, person-centered care. North Range Integrated Care staff collaborate closely with medical teams across multiple community-based clinics including Sunrise Community Health locations such as Bond Children’s Care, Monfort Children’s Clinic, Monfort Family Clinic, Adelante Clinic, Kid’s Care Clinic, Sunrise Windsor, and the Sunrise/North Range Main Center in Greeley, as well as Banner Health’s North Colorado Family Medicine in Greeley. This position includes eligibility for a monthly performance bonus.

Requirements

  • Have a Masters Degree in a Social Work, Clinical Mental Health Counseling, Addictions Counseling, Rehabilitation Counseling, or other relevant clinical degree from an accredited college of university.
  • Have a valid driver’s license and an insurable driving record.
  • Can fluently speak English/Spanish and pass a language validation
  • Can successfully complete pre-employment screening, including background check, fingerprinting, and drug and TB testing.

Responsibilities

  • Provide person-centered assessment, goal planning, and treatment for mental health and/or substance use issues.
  • Provide screening, intakes, and assessment of clients utilizing required instruments and formats within appropriate timelines.
  • Develop strength-based written treatment plans/updates and continuing care plans and make appropriate referrals.
  • Complete written discharge summaries and correspondence within established timelines.
  • Assist individuals in improving their well-being and quality of life by training in behavioral skills; educate clients to make information choices; develop client coping skills; help clients to problem solve and negotiate tasks of daily living.
  • Maintain electronic health records in accordance with agency standards; confirm coding of services rendered is ethically appropriate and results in successful billing.
  • Learn, utilize, and support fidelity of any evidence-based practices within assigned program to ensure effective interventions with clints.
