The Social Worker will serve as part of an interdisciplinary team caring for patients at CommUnityCare. This is a grant-funded position ending on May 31, 2027, with the possibility of renewal based on funding availability, work performance, and progress toward established goals. This role offers a hybrid work schedule, with certain duties performed off-campus at a personal residence located within 75 miles of a university-owned or leased property. Remote work requires reliable internet access and a suitable, distraction-free workspace. A flexible work arrangement agreement will be required upon hire. This position represents a collaborative effort between the Division of Health Social Work and the Department of Internal Medicine. The Division of Health Social Work serves as a centralized hub for social workers and other integrated health providers, fostering an environment that empowers professionals to improve the health and enhance the well-being of patients and families in partnership with patients, families, and communities. Through cutting-edge models and comprehensive services, the division creates tangible value within the healthcare ecosystem while championing whole-person care and interprofessional collaboration. Functioning as a formalized collaborative structure, the Division of Health Social Work unites interprofessional missions, cultivating synergies that drive impactful, patient-centric care.
