About The Position

The Bilingual Licensed Intake Coordinator is primarily responsible for contacting individuals referred for services within 24 hours, adhering to the FSLA no wait time policy. This role involves conducting initial intake activities and thorough assessments, completing Diagnostic Assessment reports that reflect person-centered, culturally sensitive, and trauma-informed evaluations. The assessments must include evaluations of the client’s mental status, strengths, needs, abilities, and preferences, as well as risk and protective factors. The coordinator determines the appropriate level and intensity of services needed and identifies referrals when necessary. Additionally, the coordinator is responsible for making initial contacts with various service providers, completing individualized treatment plans, and ensuring all documentation is submitted timely and accurately. The role also includes care coordination, maintaining confidentiality, and participating in team meetings and professional development trainings.

Requirements

  • Fluency in English and Spanish (speaking, writing, and reading).
  • Licensed in a relevant field (e.g., social work, counseling).
  • Experience in conducting assessments and treatment planning.
  • Knowledge of trauma-informed care and culturally sensitive practices.
  • Ability to maintain confidentiality of PHI.

Nice To Haves

  • Experience with electronic health record (EHR) systems.
  • Familiarity with FSLA protocols and standards.
  • Previous experience in a community service provider role.

Responsibilities

  • Contact persons referred for services within 24 hours.
  • Conduct initial intake activities according to FSLA protocol.
  • Complete a Diagnostic Assessment report reflecting person-centered, culturally sensitive, and trauma-informed clinical evaluation.
  • Make initial contact with the school system, primary care physicians, and previous mental health providers.
  • Complete an individualized treatment plan (ITP) within 7 days.
  • Ensure ISPs are completed with active participation of the person served.
  • Submit completed Diagnostic Assessment report and ITP for review within 24 hours.
  • Coordinate medically necessary medication evaluation/management appointments.
  • Take lead role in care coordination as privileged by the clinical supervisor.
  • Submit completed client records to the Administrative Assistant for electronic health record scanning.
  • Maintain knowledge of significant developments and trends in the field.
  • Occasionally provide outpatient therapy, crisis intervention, and other services as directed.
  • Utilize evidence-based interviewing techniques.
  • Participate in team meetings and FSLA required professional development trainings.
  • Work collaboratively with the person served and other care providers.
  • Follow all FSLA policies and procedures and maintain confidentiality of PHI.
  • Conduct oneself in a professional manner and comply with accreditation standards.
  • Maintain 15 billable hours of services per week.
  • Perform other duties as assigned.
