About The Position

Through ODHS/Self-Sufficiency program, the Health Navigator provides support to clients to enable them to fully participate in mental health, alcohol and drug, and medical assessments and treatments. Families served include those participating in the JOBS program. This is accomplished through respectful, strength-based services that create an appropriate relationship in a brief period. Health Navigators meet with clients in their home and in the community, which allows them to attend to non-verbal cues to quickly establish rapport, which enhances the supportive relationship. Health Navigators may be required to use their own vehicle to transport clients.

Requirements

  • Bachelors degree in social sciences or relevant field.
  • At least two years of experience in working with disadvantaged individuals in the provision of services.
  • Bilingual, English/Spanish (applicants will have to pass an assessment to qualify).
  • Must be familiar with community resources and crisis services and how to access them.
  • Must have respectful, open, unbiased, accepting attitude toward clients, their families, and other staff.
  • Pass a ODHS background check, which may include fingerprinting.
  • Must have a valid Oregon drivers license, reliable transportation, and auto insurance.

Nice To Haves

  • Knowledge and experience of ODHS/Self Sufficiency guidelines and expectations, as well as mental health and/or alcohol and drug issues preferred.
  • Experience working in a clinical setting, mental health or medical.

Responsibilities

  • Receive, process, and track referrals.
  • Maintain knowledge of local providers, their services, and who is accepting new patients.
  • Facilitate the connection to local providers such as; Voc. Rehab, Adapt, Compass, UCHC, South River Health, VA, Mercy.
  • Ensure releases of information(s) are completed for all programs/service involved.
  • Mentor participants through appointment scheduling process for their Mental Health/Alcohol & Drug/Medical plans.
  • Complete reminder calls/email/text for participants upcoming appointments or assigned tasks.
  • Mentor participants through arranging transportation set up process for their Mental Health/Alcohol & Drug/Medical plans. This may include providing the transportation when other available options have been attempted.
  • Gather, input, and track data for Mental Health/Alcohol & Drug/Medical plans including attendance, recommended/offered accommodations, and documentation. May include home visits/site visits.
  • Report on case in staffing(s) with Family Coaches & Employment Specialists which include a bi-monthly formal meeting as well as impromptu staffing(s) based on case happenings.
  • Monitor the Outcome Metrics
