The Health Navigator position is for a Bi-Lingual Spanish speaker, with a salary range of $45,000 - $50,000, and additional bonuses available based on performance and Spanish proficiency. This role requires the use of a personal vehicle to transport patients to appointments, and a valid Arizona driver's license is necessary. Gellert Health is committed to providing a concierge level of health navigation and advocacy to the most vulnerable and underserved populations, ensuring that all patients receive the best possible care.

Requirements

  • Full-time position Monday- Friday (8am-5pm with a 1-hour lunch break)
  • Bi-lingual Spanish speaking
  • Associate's Degree or Bachelor's Degree in a related field; or High School / GED plus demonstrated relevant experience in medical or behavioral health field
  • 21+ years old with a current Arizona’s driver’s license with 39 months clear MVR, a four-door car, and auto insurance
  • Able to drive patients to and from appointments in their personal car
  • Ability to successfully pass a criminal background check
  • Ability to successfully pass a 10-panel drug test
  • Excellent oral and written communication skills, and proficient typing skills
  • AZ Fingerprint Card
  • CPR/First Aid certification
  • TB Test/Flu Shot - Exceptions may be discussed

Responsibilities

  • Schedule healthcare appointments with patients
  • Attend appointments with patients
  • Keep track of patient’s medications and refills
  • Document all appointment and patient information in an Electronic Medical Record
  • Relay information from one provider to another for continuity of care
  • Help to prevent duplicative services and testing
  • Increase utilization of preventative services and decrease emergency department use
  • Work day-to-day on patient goals
  • Connect patients to Social Determinants of Health (SDOH) providers
  • Improve day-to-day safety of patients' living environment
  • Transport patients to critical appointments in personal vehicle
