About The Position

This position is responsible for delivering education, outreach, and in-person assistance to community members accessing the health care exchange through Connect for Health Colorado. This position is also responsible for processing Health First Colorado and Child Health Plan Plus (CHP+) applications and all related paperwork through Hilltop’s Eligibility Assistance Site (EAP). Utilize bilingual language skills to support clear and effective communication with staff, clients, and community members as needed. This role does not include formal translation or interpretation responsibilities.

Requirements

  • High School Diploma or equivalent.
  • Valid Colorado Driver’s License

Responsibilities

  • Provide complete, fair, and impartial information about health care coverage options to consumers. Including explaining services and options for coverage through the Health Insurance exchange, Health First Colorado Programs and CHP+.
  • Meet with customers in person or virtually to review their household demographics, including household size and income to determine program eligibility.
  • Meet confidentiality and privacy standard set by Connect for Health Colorado and Health Care Policy and Financing (HCPF).
  • Maintain expertise in eligibility, enrollment and program specifications.
  • Assist customers to set up appropriate system accounts, and educate them to allow customers to selfnavigate websites in the future.
  • Provide education regarding different aspects of insurance, including health literacy education, rights and responsibilities and reporting of household changes and their effect in eligibility status.
  • Assists customers with appeals, grievances, and complaints and advocate as needed.
  • Make referrals to community partners as necessary to ensure client’s needs are met.
  • Follow up with clients after initial meeting to ensure completion of enrollment, answer questions about life event changes and other related questions regarding health insurance coverage.
  • Process medical assistance applications, redeterminations and case changes through the Colorado Benefits Management System (CBMS) received via fax, mail, or walk-ins.
  • Explain eligibility requirements and rules regarding the more complex Health First Colorado including Medicare Savings Program and Buy-in programs.
  • Track and record activities in multiple databases and tracking systems.
  • Actively participate in outreach, community enrollment events and special projects as needed. Including presentations to various community agencies and groups. Work with community partners to increase access to Health First Colorado services, including being the first point of contact regarding application status and eligibility questions.
