About The Position

Eliot Community Human Services is seeking a part-time bilingual Family Partner who will support families of youth with special needs, particularly those with mental health challenges. The role involves engaging caregivers in activities that align with treatment goals, providing education and support, and facilitating connections to community resources. The ideal candidate will have lived experience as a caregiver and be fluent in English and one or more additional languages such as Spanish, Arabic, Haitian Creole, or Portuguese.

Requirements

• Bilingual in English and Spanish, Arabic, Haitian Creole, or Portuguese.
• 1 year of experience navigating child and family service systems.
• Lived experience as a caregiver to a youth with special needs, preferably mental health needs.
• Strong communication skills.

Responsibilities

• Engage the parent/caregiver in activities in the home and community to address goals on the youth's treatment plan.
• Assist parent/caregiver in meeting the needs of the youth through education, support, coaching, modeling, and guidance.
• Teach parent/caregiver how to network and link to community resources and treatment providers.
• Support parent/caregiver in advocating for services and resources for the youth.
• Guide and foster empowerment, including linkages to individual, peer/parent support, and self-help groups.
• Participate on the Care Planning Team (CPT) and attend CPT meetings.
• Participate in discharge planning.
• Meet with providers and attend all relevant meetings such as IEP, hospital discharge, and treatment team meetings.
• Provide telephone support and outreach to parent/caregiver.
• Make regular telephone and face-to-face contacts with collateral providers.
• Provide transportation to parent/caregiver as appropriate.
