This position coordinates the care and serves as ECM Point of Contact/Lead Care Manager for the Full Circle Health ECM qualifying youth. An ECM Manager is a Lead Care Manager specializing in Enhanced Care Management, providing comprehensive support to enrolled individuals. They act as a central point of coordination, connecting members with vital resources such as housing, food, and childcare, while also collaborating with diverse healthcare providers to ensure seamless and coordinated care.
Requirements
- Master’s degree in social science plus one (1) year of experience in behavioral health. Bachelor’s degree in social science plus two (2) years of experience in the behavioral health field. Associate’s degree in social science plus three (3) years of experience in the behavioral health field.
- Valid California Driver's License.
- Must meet Agency driver eligibility criteria.
