About The Position

As we continue to grow across Western New York, we’re looking for a Bilingual Community Health Worker (Spanish/English) to support patients and families in Genesee County. This is a hands-on, community-focused role where you’ll serve as a trusted link between patients, providers, and essential resources—helping individuals navigate the healthcare system while addressing real-life barriers to care. From supporting our local agricultural population to connecting families with critical services, your work will directly improve health outcomes and strengthen our communities.

Requirements

  • High school diploma or equivalent
  • Bilingual Spanish/English required (critical to supporting our patient population)
  • Community Health Worker (CHW) certification or willingness to obtain
  • Ability to support a wide range of responsibilities—from patient navigation and scheduling to case management, follow-up, and community engagement
  • Strong interpersonal, communication, and organizational skills
  • Comfort working with diverse populations while maintaining professionalism and boundaries

  • Experience in community health, social services, healthcare, or outreach preferred

Responsibilities

  • Help patients navigate the healthcare system, including scheduling, confirming, and coordinating appointments
  • Conduct needs assessments and identify barriers such as transportation, language, literacy, and financial challenges
  • Provide Spanish interpretation and written translation within your scope
  • Connect patients to community resources like food, housing, insurance, and medication assistance
  • Coordinate transportation and other access-related services
  • Educate patients on health promotion and disease prevention
  • Support outreach efforts and build relationships within the community
  • Collaborate closely with clinical and care teams to ensure coordinated, patient-centered care
