The Preventive Case Manager is responsible for providing counseling and case management services to community youth and their families designed to prevent long term out of home and foster care placements. Specific responsibilities include but are not limited to: maintaining a positive relationship with the Department of Social Services, completing necessary paperwork and reports, completion of Family Assessment and Service Plans as required by State of New York in a timely manner, providing needed preventive services to children, youth and their families including but not limited to individual and family counseling, ability to engage adolescents and families in needed intervention services, attending all relevant community based meetings for children, youth and families, participating in weekly staff meetings and case conferences, attending specific case conferences at the Department of Social Services, sharing on-call coverage, and other duties deemed necessary by the Program Supervisor.
