The Behavioral Health Partners of MetroWest (BHPMW) provides collaborative care coordination and wellness goal support to individuals with behavioral health and/or substance use needs, based on the recent transformation of Mass Health’s service delivery model. The partnership is built on a collaboration among five community-based provider agencies: Advocates, Family Continuity, South Middlesex Opportunity Council (SMOC), Spectrum Health Systems, and Wayside Youth and Family Support Network, throughout MetroWest, Central and Northern Massachusetts. The Care Coordinator provides information, referrals, and care coordination to people seeking behavioral health (BH), social services, and primary healthcare needs. Works in collaboration with the Clinical Operations Manager and other team members in the development of systems, services, and internal and external relationships. BHPMW and its MetroWest Care Connection hub serves as a single point of entry into the full continuum of BH and social services operated by the legal partnership of Advocates, South Middlesex Opportunity Council (SMOC) and Wayside Youth and Family Support Network (Wayside).
