About The Position
Responsible for working with adults with mental health and substance use diagnosis in community and home environments. Provides a broad range of social services to clients and participates in treatment team planning. Serves as a liaison for social services and linkage to community resources. Performs case management duties, education, and skill building.
Requirements
- For a level III, High school diploma and 3+ years customer service experience, preferably in human services, with one year case-management experience; bachelor’s degree preferred.
- For a level IV bachelor's degree in helping field.
- Must be at least 21 years old to apply.
- Must possess a valid driver’s license and have a clean driving record with less than 6 points.
- Knowledge of diagnostic impressions or criteria.
- Knowledge of how client advocacy system operates.
- Knowledge of various therapeutic models and interventions as directly related to area of assignment.
- Knowledge of crisis intervention techniques.
Nice To Haves
- Preference for experience with or knowledge of trauma informed, resilience-oriented care.
Responsibilities
- Will work Monday-Thursday @ 10-hour days
