About The Position

Assistance Plus is seeking an individual to fill the role of Nurse Care Manager. This is a unique opportunity for a nurse looking for a more standard schedule -- no weekends, no nights, and no on-call! Much of this position is held within the office setting. Position Overview: The Behavioral Health Home Organization Nurse Care Manager (BHHO NCM) carries out medical functions to all clients receiving services from the BHHO team. Medical functions include basic psychiatric health assessments, treatment, monitor medications, education, and coordination of health care provided to clients in the community. The BHHO NCM will also be responsible for assuming lead role in developing and directing rehabilitation services to a designated caseload of clients. These responsibilities include ensuring face-to-face contact is completed to ensure safety and quality treatment is being provided according to all regulations and laws pertaining to licensure in Maine with the State Board of Nursing, codes of ethics, MaineCare rules and regulations, HIPAA, and Agency policies and procedures and is within scope of practice and the client’s plan of care.

Requirements

  • Proof of vehicle insurance at $50K/$100K and a valid Maine drivers license are required
  • Must be 18 years of age or older
  • Associate's or Bachelor's Degree in nursing is required
  • Minimum of three years clinical nursing experience is required
  • Must hold a current license to practice in the State of Maine as a Registered Nurse (RN), Advance Practice Nurse (APRN), or Clinical Nurse Specialist (CNS)
  • Intermediate level computer experience
