The BHASO Care Coordination Lead will organize, collect, review and report behavioral health, social determinant of health and/or physical health information through Individual phone in-reach and outreach, while demonstrating multicultural responsiveness and effective communication skills with Medicaid members. This position follows established safety protocols in the community setting, as well as established preventive and disease management programs for health promotion and education. Deliver culturally responsive information regarding the availability of health and community resources that will reduce barriers to care. Connect Individuals to payers and services that the Individual is eligible for. Work collaboratively with the Regional Accountable Entities and the state. Lead care navigators and other team members involved in direct or indirect coordination of care.

Responsibilities

  • Serves as a consultant to care coordination teams
  • Provide direct care navigation, coordination and management for the BHASO
  • Respect confidentiality and maintain confidences as described in the UHG Employee Handbook
  • Participate in Interdisciplinary care team meetings as indicated
  • Serve as community liaison and maintain relationships with key individuals in the community
  • Coordinate and perform duties of communicating the mission and role of the organization to community based organizations
  • Conduct Individual assessments
  • Assess the changing needs and condition of the Individual and family
  • Document assessments, Individual/family response to care coordination interventions
  • Educate and assist identified Individuals about behaviors that can enhance their health
  • Facilitate connection and access to identified treatment needs
  • Develop a plan of management associated with health care goals for each Individual
  • Develop and maintain a report system for outcomes
  • Communicate Individual issues requiring interventions to appropriate departments and providers
  • Participate in staff meetings, case conferences and in-services

Requirements

  • Master's degree in a behavioral health field such as Social Work, Counseling, Pastoral Counseling, Psychology
  • 1+ years of case management experience
  • 2+ years of progressive related experience working with diverse populations, community or faith-based organizations
  • Knowledge of health education, motivational strategies, and an empathetic manner working with the underserved
  • Driver's License and access to a reliable transportation
  • Ability to travel up to 25%
  • Resident of Colorado
