Under the direction of the Behavioral Health (BH) Director, the BH Provider will provide direct services including individual (child, youth, and adults), couple, family and group therapeutic services to Sonoma County Indian Health Project, Inc. (SCIHP) patients. In the BH department and as needed in the Medical Department as part of SCIHP’s integrated warm hand offs in the Medical Department, and provide treatments that include screening tools, managing panels, providing time bound therapy and solution focused therapy techniques. You will collaborate with Supervising LCSW to determine the best assessments, treatment, and discharge plans on a case-by-case basis and by patient need. Responsible to coordinate the activities and objectives of the DHCS BH Program including plans to improve physical and behavioral health outcomes, care delivery efficiency, and patient experience by expanding SCIHP’s integrated care services. The BH Provider will be involved in an integrated system of care in development of a holistic and trauma-informed care that is culturally responsive and culturally appropriate.
